Attaching a Document to Patient’s/Subject’s File

   If you don’t have access to the following screens, please contact the Admin to ask for the appropriate access rights.

 

From the List of patients

To attach one or many document(s) to a patient’s file, go to the List of patients. Find the patient in the list.

On the right of the table, find the icon to attach a document.

 

> If the icon has a cross in the right corner, it means that no document has been attached to the patient’s file.

 

> If the icon is greyed out in the right corner, it means that at least one document has been attached to the patient’s file.

Click on the icon to attach a document.

From the patient’s file

Go to the patient’s file and the field where you need to attach the document.

Answer ‘Yes’ to the question and click on the button ‘Upload file’.

Attach a document

After clicking on the icon or on the button ‘Upload file’, a pop up window will appear on the screen, click on the button ‘Add a new file’.

Then select in the drop down list the type of file you want to attach, add some comments if necessary, browse the file, then submit.

If you want to attach multiple files, you have the option at the top of the window.

 

View the list of documents attached

To see all the documents attached to a patient’s file, you can either click:

  • on the icon in the list of patient
  • or on the button ‘Click here to the file list’ in the patient’s file

A pop up window will appear with the list of all the documents attached.

You will see information on:

  • In which section of the eCRF the file has been attached
  • When it has been attached
  • The type of document
  • The size of the document
  • An option to modify the document – only the type of document and comments
  • An option to delete the document
  • A link to review the document
