If you don’t have access to the following screens, please contact the Admin to ask for the appropriate access rights.
Once you’ve added a new patient in the SMART platform, you will need to complete all the information relevant to this patient.
Find your patient in the List of patients and click on your patient ID to access their information.
In the patient file, follow the steps to complete the information.
The fields which are greyed out are automatically completed by the platform.
The fields marked with a ‘*’ symbol are to be completed manually and are mandatory.
For some fields (numbers, dates) boundary can be indicated to help you to fill in the field.
To save your progress, you will need to click on the ‘Save and continue’ button located at the bottom of each page. For security reasons, you will sometimes need to enter your password to save the page.

You can navigate through the patient’s file by seeing each of the eCRF sections in the left Menu.
Please note that these sections might differ from one study to another.

Once the page is signed, you can make changes and save them by clicking on « save and continue ». If you do not make any changes on the page you can click on « Next page » to continue.
